Can medications like Ozempic, Wegovy, Mounjaro and Zepbound increase the risk of sudden vision loss? Recent studies have raised concern about an association between semaglutide and nonarteritic anterior ischemic optic neuropathy—NAION, a condition that can cause sudden, painless and sometimes permanent vision loss.


But the story is much more complicated than the headlines suggest.


In this special Eye-Q episode, neuro-ophthalmologist Dr. Rani Banik explains:

Importantly, do not stop a GLP-1 medication because of this episode. These drugs can provide substantial metabolic, cardiovascular and other health benefits. Individual risk and benefit should be discussed with the prescribing clinician.

**IMPORTANT CONTEXT: This is a rapidly evolving area of research. The studies discussed in this episode primarily show an association between semaglutide exposure and NAION; observational studies cannot by themselves prove that the medication caused an individual patient's optic neuropathy.


The size of the reported association has also varied substantially. The original 2024 neuro-ophthalmology referral-center study reported a relatively large association, while a subsequent analysis of 37.1 million adults with type 2 diabetes found a much smaller increase and an incidence of about 14.5 NAION cases per 100,000 person-years among semaglutide users. JAMA Network


Also, the strongest signal to date is for semaglutide. We should not automatically assume that every GLP-1–based medication carries exactly the same risk.


Most importantly: do not stop Ozempic, Wegovy, Mounjaro, Zepbound or another prescribed medication without speaking with your treating physician.
